How CCMDS reviews and publishes information

The aim is to document recurring regulatory problems without turning allegations into facts by repetition.

Unverified report

Received but not yet checked against documents or independent sources.

Under review

Evidence being assessed

Chronology, records and applicable rules are being examined.

Verified

Core facts corroborated

The principal factual claims are supported by reviewed records.

Escalated

Institution contacted

A formal representation, correction request or right-of-reply invitation has been sent.

Resolved

Outcome recorded

A corrective action, explanation or satisfactory result has been documented.

Closed

No further action

The matter is withdrawn, unsupported, duplicated, outside scope or otherwise concluded.

What we verify

  • Identity or confidential pseudonymous verification
  • Dates and chronology
  • Authenticity and relevance of documents
  • Applicable regulations or published guidance
  • Whether the institution had an opportunity to respond
  • Whether publication could expose patients or third parties

What we reject

  • Threats, abuse or discriminatory content
  • Unredacted patient records
  • Doxxing or private contact details
  • Manipulated or misleading documents
  • Claims that cannot be separated from speculation
  • Requests for retaliation against named individuals

Publication format

Each report should display its status, issue category, jurisdiction, authority, date first published, date last updated, evidence reviewed, institutional response, outcome and correction history.

Timestamp policy: Every material update should show the date and nature of the change.
